Why 20 Minutes? The Reason Sustained Warmth Takes Time

When you place a warm mask over your eyes, the heat you feel on the outside of your eyelid is not the same temperature reaching the inner eyelid and meibomian gland area. Heat transfer through tissue takes time.

Outer warmth is not inner warmth

The eyelid is a layered structure. Warmth applied externally must pass through the outer eyelid tissues before influencing the inner eyelid and the meibomian glands embedded within the tarsal plate.

TFOS notes that the temperature at the treatment surface is not the same as the temperature at the inner eyelid or gland area, and that the ideal combination of warming temperature and duration has not been definitively established for every device or method.¹

What the research shows

In one randomized human warm-compress study, researchers compared three compress methods and measured both outer and inner lower-eyelid temperatures.

In the optimized-contact group, the outer lower eyelid reached its maximum temperature after approximately four minutes, while the inner lower eyelid reached its maximum temperature after approximately 20 minutes. This shows that the external and internal eyelid surfaces can warm at different rates.²

The same study found that an inner lower-eyelid temperature of at least 40°C was achieved after four minutes under its carefully controlled optimized method. The authors also emphasized close contact, frequent reheating, and a compress temperature near 45°C. Their findings should therefore not be interpreted as proof that every device requires exactly 20 minutes.

TFOS similarly states that the temperature and duration needed to warm obstructed meibomian material have not been definitively established. It also clarifies that the frequently discussed 40°C target refers to the palpebral conjunctiva and gland — not the treatment surface or external eyelid skin.¹

This is an evidence-informed protocol choice — not a claim that every shorter session is ineffective or that UNCLOGIC™ reproduces the exact temperature profile measured in the cited study.

Why not 15 minutes?

Fifteen minutes may still provide useful warming, and Eye Comfort Care does not claim that it is ineffective. However, research shows that heat transfer depends on the warming method, temperature retention, contact with the eyelid, and application time.

In one human warm-compress study, a 15-minute compress without reheating reached a maximum inner lower-eyelid temperature of about 38.8°C. Under a carefully controlled method with close contact and frequent reheating, the inner lower eyelid continued warming and reached its maximum temperature at approximately 20 minutes.²

Eye Comfort Care selected a monitored 20-minute routine to provide a consistent, sustained session with additional time for warmth to transfer through the eyelid. This is a protocol choice — not proof that 15 minutes cannot help.

Why 20 instead of 15?
Because the outside of the eyelid can warm before the inner eyelid reaches its highest temperature. Our monitored 20-minute routine provides sustained warmth across a complete session — without claiming that shorter routines never work.

Important safety guidance

Always remain awake during your warming session. Do not use a powered warming mask while sleeping or resting unsupervised.

After warming, blink slowly and completely. Do not rub, squeeze, or manually express the eyelids unless specifically instructed by a qualified clinician.
